(Nov. 13, 2025) – In its commitment to high-quality care with a focus on safety, WellSpan is honored to achieve four “A” safety grades for fall 2025 from The Leapfrog Group, an independent national watchdog organization focused on patient safety. WellSpan Ephrata Community Hospital, WellSpan Good Samaritan Hospital, WellSpan Waynesboro Hospital and WellSpan York Hospital all received the top rating.

WellSpan’s approach to quality is rooted in organization-wide management practices that promote a strong culture of safety through evidence-based practices and knowledge sharing. WellSpan’s quality and patient safety policies are guided by the principles of STEEEP: Safe, Timely, Effective, Efficient, Equitable, and Patient/Person-Centered care for every patient, every time, under every circumstance. These programs involve ongoing evaluation of clinical processes, identification of areas for improvement and the implementation of evidence-based practices to optimize patient outcomes.
The Leapfrog Group assigns an “A,” “B,” “C,” “D,” or “F” grade to general hospitals across the country twice a year based on more than 30 national performance measures reflecting errors, accidents, injuries, and infections, as well as systems hospitals have in place to prevent harm.
The Leapfrog Hospital Safety Grade is the only hospital ratings program based exclusively on hospital prevention of medical errors and the resulting harm to patients. The grading system is peer-reviewed, fully transparent and free to the public.
