Quality Skilled Nursing care, Respite Care, Secured Dementia Care. We offer Transitions, a 39 bed dedicated unit for Short Term Rehabilitation. At Transitions, we offer 7 day/week therapies. We also offer a transition of care program: scheduled follow up calls post discharge to monitor progress at home. We offer Registered Dieticians, a Wound Care RN, as well as Social Work and Chaplain.
